As you know, I do the same. And I don't even think it's about guilt at skipping work, because obviously I don't have a set schedule and can take time off pretty much whenever I feel like it, and no one would know the difference. But I will spin all kinds of stories with alternate explanations as to why certain symptoms are happening, and avoid taking meds when I should just give up and take them.
It annoys me that I do this. I could have taken a pill at 8 and waited until 10 to come to work. I would be fine by now. But No... I had to ignore it and I took the pill at 11 and who knows if it will work. :(
